Congenital deafness in cats: why white cats with blue eyes carry the highest risk

The dominant white gene (W) is linked to congenital hearing loss. White cats with blue eyes have a 60 to 80 percent chance of unilateral or bilateral deafness. BAER testing is the only objective diagnosis, and a deaf cat lives a near-normal life with a few home adjustments.

Congenital deafness is one of the best-documented inherited conditions in the domestic cat, and it tracks one specific gene: the dominant white gene (W) that produces an all-white coat. Not every white cat is deaf, but the rate runs far above the feline average. The single strongest predictor is eye color. A white cat with two blue eyes carries a 60 to 80 percent chance of hearing loss in one or both ears, while a white cat with green or gold eyes drops to roughly 17 to 22 percent. The only objective way to confirm it is a BAER test, which catches the one-sided deafness an owner would never notice at home. A deaf cat lives a near-normal life indoors with a handful of adjustments built around sight and vibration instead of sound.

How the W gene drives deafness

The dominant white gene is tied to the development of the inner ear during the embryonic stage. The same genetic variant that suppresses pigment in the coat can also block the normal development of the organ of Corti, the structure in the cochlea responsible for hearing. When pigment-producing cells fail to migrate into the inner ear, the specialized cells there degenerate in the first weeks of life, and the hearing loss that follows is permanent.

This is why the link is so tight to white coats specifically, and tighter still to blue eyes. The blue iris in these cats comes from the same lack of pigment, so eye color works as a visible marker for how completely the W gene has suppressed pigmentation, including inside the ear.

Risk by phenotype

Epidemiological data on white cats (Strain 2007; Cvejic et al. 2009) line up the risk by coat and eye color:

The odd-eyed pattern is the most telling. When deafness shows up in a cat with one blue eye and one gold eye, it usually affects the ear on the same side as the blue eye, which is the side where pigment is most suppressed.

Breeds with higher prevalence

Any breed with a high proportion of solid-white cats carries elevated risk. The trait follows the white coat, not the breed itself:

  • Turkish Angora, classic white lines.
  • Turkish Van, where the van pattern leaves most of the body white.
  • Khao Manee, a Thai breed bred specifically for an all-white coat and often odd eyes.
  • Persian and British Shorthair in their white varieties.
  • White domestic shorthairs, the most common white cat in US shelters.

A white kitten from any of these lines, or a random-bred white kitten with blue eyes, is a candidate for a hearing check before adoption.

Diagnosis: the BAER test

The BAER test (Brainstem Auditory Evoked Response) is the only objective method. It records the electrical response the brainstem generates to a sound stimulus through small electrodes placed under the skin, so it measures actual hearing rather than a behavioral reaction the cat could fake or compensate for. Its real value is catching unilateral deafness, which is invisible at home because a cat that hears in one ear behaves like a normal cat.

The test runs under light sedation, takes 30 to 60 minutes, and needs specialized equipment that not every general practice carries. In the US it is most often available through veterinary teaching hospitals and board-certified specialists; ask your vet for the nearest referral center.

When to do it:

  • Kittens from white lines, ideally before adoption, so the new owner knows the hearing status.
  • Breeding cats in predisposed breeds, which responsible breeders screen as standard practice.
  • Any white cat showing suggestive signs: not responding to its name, sleeping through loud noises, or startling only when it sees movement.

Living with a deaf cat at home

A deaf cat has a full, normal life with a few adjustments that lean on sight and vibration:

  1. Vibration and visual cues instead of sound. The cat feels footsteps through the floor and registers a person's presence by movement and air currents rather than voice.
  2. No free outdoor access. A deaf cat cannot hear a car, a dog, or a predator approaching, so the safe choices are supervised time outside or an enclosed catio.
  3. Wake it within its field of view. Approach where the cat can see you and avoid touching it suddenly from behind, which can trigger a defensive startle.
  4. Call with gestures or light. A hand signal or a flicked light switch replaces calling the cat's name; many owners train a consistent flashlight blink for mealtime.
  5. A feline companion helps. A second hearing cat aids socialization and acts as an early-warning system for events the deaf cat cannot detect.

Acquired versus congenital deafness

Hearing loss with no connection to the W gene has other causes worth ruling out, especially when a non-white or older cat goes deaf:

  • Chronic otitis that reaches and damages the inner ear.
  • Drug toxicity, particularly some aminoglycoside antibiotics, which can be ototoxic.
  • Age-related hearing loss (presbycusis) in very old cats.
  • Tumors of the ear canal or middle ear.

Congenital deafness is present from birth and does not progress. A cat that loses hearing later in life points toward one of these acquired causes and deserves a workup.

Implications for breeding

Responsible breeders of predisposed breeds (Turkish Angora, Turkish Van, Khao Manee, white Persian) build hearing screening into their program:

  • BAER test every kitten before placement, with results disclosed to the buyer.
  • Avoid breeding two white cats together, which raises the odds of the highest-risk genotype in the litter.
  • Full transparency with the adopter about each kitten's hearing status, including unilateral cases that look normal at home.

The CFA and TICA breed standards do not penalize white coats or blue eyes, so the burden of screening falls on the breeder's own ethics rather than the show ring.

Common questions

My white cat seems to hear fine. Could it still be deaf in one ear?

Yes, and this is the exact reason BAER testing exists. A cat with normal hearing in one ear responds to its name, startles at noises, and behaves like any hearing cat, so unilateral deafness is effectively undetectable by observation. Only the BAER test separates one-eared from two-eared hearing.

Are all white cats deaf?

No. Even in the highest-risk group, white cats with two blue eyes, the rate is 60 to 80 percent, not 100 percent. White cats with green or gold eyes fall to roughly 17 to 22 percent. Coat and eye color shift the odds; they do not guarantee the outcome either way.

Can deafness be cured or treated?

Congenital deafness is permanent because the inner ear failed to develop, and there is no surgical or medical fix. The goal is management, not cure: a safe indoor environment, visual and vibration-based communication, and protection from the outdoor hazards a deaf cat cannot hear coming.

Should I ask for a hearing test before adopting a white kitten?

If the kitten has blue eyes, a documented BAER result is worth requesting from a responsible breeder. For a shelter kitten where no test is available, plan to live with it as an indoor cat and watch for the suggestive signs; a vet referral for BAER testing settles the question. A deaf cat is a fully adoptable, affectionate pet, so the goal of testing is informed care, not screening the cat out.
